Bale scored a magnificent overhead kick to put Real 2-1 after Karim Benzema's opener was cancelled out by Sadio Mane.
Liverpool had lost their top scorer Mohammad Salah to injury after a challenge from Real's Sergio Ramos midway through the first half.
It was a nightmare performance for the German when he threw the ball to Benzema for Real's first goal. Mane equalised just four minutes later before Bale came off the bench to score the stunner and add another after Karius spilled Bale's powerful shot into the net to seal Real's victory.
The victory sealed Real's record 13th Champions League title and coach Zinedine Zidane's third triumph in three years. It was disappointment for Liverpool manager Jurgen Klopp after losing his third successive final since taking over at Anfield.
